Job Description Summary
The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Key Responsibilities
- Perform initial patient and family assessments, including physical exams and medical history.
- Develop and update care plans with preventive, therapeutic, and rehabilitative actions.
- Administer medications and treatments as prescribed.
- Provide patient/family education and counseling.
- Facilitate discharge planning and act as Case Manager when assigned.
- Collaborate effectively with physicians and community health resources.
- Supervise home health aides—evaluate them every two weeks.
- Manage on-call rotation, and coordinate equipment, supplies, and related services.
Qualifications
- Graduate of accredited nursing program with 1–2 years acute care experience.
- Active RN license in State of California and current CPR certification.
- Bachelor’s degree and prior home health experience preferred.
- Excellent communication, observation, and problem-solving skills.
- Physically able to lift, transfer, and position patients.
- Valid driver’s license and reliable, insured vehicle.
Salary Range: $100,000 to $130,000 Plus Benefits

Job Description Summary
The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and collaborating with the care team to achieve positive patient outcomes.
Key Responsibilities
- Provide direct patient care as defined in the State of California Nurse Practice Act.
- Implement plans of care as directed by the registered nurse.
- Document patient care accurately and in a timely manner.
- Educate patients and families on diagnoses and care plans.
- Assist the RN or physician with specialized procedures and patient self-care education.
- Coordinate home health services by reporting additional needs to the RN or supervisor.
- Use equipment and supplies efficiently and effectively.
- Participate in ongoing personal and professional growth.
- Perform additional duties as requested by the supervising RN.
Position Qualifications
- Graduate of an accredited practical nurse or vocational nursing program.
- At least one (1) year of professional nursing experience in home health or medical/surgical care (three years preferred).
- Current LPN/LVN license in the State of California of practice.
- CPR certification (current and maintained).
- Strong verbal, written, and organizational skills.
- Licensed driver with an insured and reliable vehicle in good working condition.
Salary Range: $70,000 to $90,000 Plus Benefits
