Registered Nurse – Home Care

About the Role
The registered nurse plans, organizes, and directs home care services with a focus on community health education. This role leverages community resources to meet the needs of individuals and families in their homes.
Patient Care Responsibilities
- Complete initial assessments of patients and families, including exams and medical history.
- Regularly re-evaluate nursing needs and update care plans.
- Develop individualized care plans with therapeutic, preventive, and rehabilitative actions.
- Administer medications and treatments as prescribed.
- Educate patients and families and provide counseling.
- Coordinate discharge planning and serve as Case Manager when assigned.
Communication
- Prepare clinical notes and update physicians regularly.
- Report changes in patient condition and obtain necessary orders.
- Coordinate with community health resources as needed.
Additional Duties
- Participate in on-call rotation.
- Arrange necessary equipment, supplies, and services.
- Supervise and evaluate home health aide performance every two weeks.
Qualifications
- Graduate of an accredited nursing school; 1–2 years of acute care experience.
- Current RN license and CPR certification.
- Bachelor’s degree and home health experience preferred.
- Strong communication, observation, and problem-solving skills.
- Physically able to lift, transfer, and position patients.
- Licensed driver with insured, reliable transportation.
